Confidentiality Statement: This information is being requested on a voluntary basis. However, some information is required in order to determine eligibility. All information disclosed will be used only by those persons related to the program and who are on a need to know basis

Who: All children who are 3 or years old on or before July 1, 2020.

How:  All applications will be stamped with the date they are received in the Office of Elementary Education. This date will be used to place children in preschool on a first come, first served basis.

All applications are put through a selection process once received.

When: Family Sevice Workers will contact you to set-up upon enrollment appointment. 

**At the time of actual enrollment, parents must present evidence of:

• Up-to-date shot record a birth certificate from the Department of Vital Statistics in the state where the child was born (hospital or county birth certificates are not accepted).
• Current WV Health Check form from the child's heath care provider
• Cerification of a dental exam within the last 12 months
• Proof of income for Head Start eligibility.
